This patient has suffered from vertical root fracture of tooth 21 carrying hopeless prognosis requiring extraction and replacement. He has elected for the replacement option of a fixed implant-supported screw-retained crown.
To optimise soft tissue response, we have carried out atraumatic extraction and carefully preserved the buccal plate and all crestal bone.
This was followed by immediate implant placement with minor soft tissue grafting to maintain the buccal profile in the long-term.
After a standard 3 months of healing and confirmation of successful osseointegration using resonance frequency analysis, we provided the patient with a fixed screw-retained porcelain fused to gold crown.
Not only have we achieved a good shade match of the tooth but also achieved excellent preservation and maintenance of the soft tissue architecture.
